Nearly everyone starting Ozempic, Wegovy, Mounjaro, or Zepbound feels something. Here is what is normal, what is manageable, what is not, and the practical steps that make the difference.

The single biggest reason people stop a GLP-1 is not that it failed to work. It is that the side effects arrived faster than the support did. Almost all of it is preventable with slower titration and a physician who answers the message the week it matters.

The common ones

Nausea, constipation, diarrhea, reflux, burping, early fullness, and fatigue account for the large majority of what patients experience. They are dose-related, which means they typically spike in the first week or two after starting and after each increase, then ease as your body adapts. They are also the reason the titration schedule exists — the escalation is designed to give your gut time to catch up.

The one nobody warns you about: muscle loss

A meaningful share of rapid weight loss on any GLP-1 can be lean mass rather than fat. Losing muscle lowers your resting metabolic rate, which makes regain easier the moment you stop — the exact outcome you were trying to avoid. Protecting muscle is not optional, and it is not complicated: hit a real protein target every day, lift something heavy two to three times a week, and keep the rate of loss reasonable rather than dramatic.

The serious but uncommon risks

Pancreatitis, gallbladder disease, kidney injury from dehydration, and worsening of diabetic retinopathy are all documented. Both semaglutide and tirzepatide carry a boxed warning about thyroid C-cell tumors observed in rodents, and neither is appropriate with a personal or family history of medullary thyroid carcinoma or MEN2.

Call your clinician promptly for severe abdominal pain radiating to the back, repeated vomiting, signs of dehydration, jaundice, or new vision changes.

When side effects mean the plan is wrong

Side effects that persist at a stable dose for more than a few weeks, or that keep you from eating and drinking adequately, are information rather than something to endure. They usually mean the dose escalated too quickly, the medication is not the right fit for your physiology, or the nutrition plan around it needs work. All three are fixable — but only if someone is actually watching.
